PRIMARY FUNCTION: Screens referrals for appropriateness and performs

admissions to facilitate the transition of the patient/family into the hospice/home

care program.

QUALIFICATIONS:

1. Current state license as a registered nurse.

2. A minimum of three (3) years nursing experience, preferably with hospice or home care.

3. A thorough understanding of Hospice and Home Care

Regulations (federal, state, and local government) preferred.

4. Ability to work a flexible schedule.

5. Must have a valid drivers license and automobile liability insurance.

TRAINING: Orientation to Hospice, Home Care Services and any other

training that may be necessary to complete the assigned duties effectively.

DUTIES:

1. Primary responsibility for taking referrals to hospice and home care programs and providing follow-up on those “Not Taken Under Care.”

2. Makes field visits to evaluate appropriateness, determine eligibility for services, and present hospice or home care program.

3. Insures communication of assessment and reassessment

findings to attending physician.

4. Makes field visits to perform admission procedures
